The CLPNM will accept scans or photographs of the following documents submitted in support of an application for registration, provided that they meet the CLPNM’s criteria:
- Government-Issued Identification

New Identification Requirements
When you submit identification documents to the CLPNM in support of your application for registration, they must conform to the CLPNM’s Mandatory Identification (ID) Requirements.
Note that the CLPNM’s ID requirements changed in August, 2026. Among other changes, the photocopy of your identification must now be witnessed.
If you are an applicant, student practical nurse, or graduate practical nurse, and have previously submitted identification to the CLPNM, you may be required to submit a new photocopy of your identification that conforms to the new requirements before you will be eligible for licensed practical nursing (LPN) registration.
